Your First Visit
Read these frequently asked questions to learn what to expect at your first visit to our office.
What will happen during my first visit?
On your first visit to our office, please arrive about 10 minutes early so that you have enough time to fill out our New Patient forms.
Your first appointment will be approximately 90 minutes in duration.
Your dentist will review your health history and complete a comprehensive examination of your gums, teeth, bite and jaw, which will include an oral cancer screening and dental x-rays. This will allow your dentist to develop a long-term dental treatment plan for you.

How can I book an appointment?
To book an appointment, please contact our office. If you like, you can also book your next appointment while you're here at the office.
If you experience dental emergency, please call us right away and we'll make room in our schedule to see you.
How do I cancel or reschedule an appointment?
To cancel or reschedule an appointment, please call us or visit us in person, rather than emailing.
Please let us know as soon as possible, so we can open up the space in our schedule for another patient.
Do you offer appointment reminders?
In the days before your appointment, you will receive automated appointment reminders from us by either phone or email.
If you must cancel or reschedule an appointment, please call us as soon as possible to let us know.
Do you offer direct insurance billing?
Our office accepts most insurance plans and we are more than happy to submit patients claims to their insurance on their behalf, however patients are responsible for paying for their appointments upfront in full with a speedy reimbursement to them from their insurance. Reimbursement depends on their insurance plan and what they are covered for.
Please bring your insurance information with you to your first appointment with us and we will be happy to submit your claim to your insurance company.

Your cooperation in completing this questionnaire is essential to provide you with safe and appropriate dental care. All information is strictly confidential. If you are unable to complete it, please arrive 10-minutes early for your appointment, as a member of our team will be able to assist you with the completion of this form.
*Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
